And yes, it did happen to me once. I dug a tunnel straight down in Minecrat on my first night on Survival mode. Until I saw lava. And Du how deep that means! I didn't have a ladder, and I was like 20 blocks down oder something, so no use using the jump-then-place-block-below-you-trick. Don't blame me, it was pitch dark and I couldn't see anything. And I was desperate.
